The Participants Database plugin doesn't only come with a backend management module as you'd expect, but it also includes a participation form for the frontend of the site.

Basically the plugin can be used as a fully functional event management tool, the form on the frontend allowing users to register and confirm their participation to an event, while the backend interface will allow webmasters to manage the events and the attending persons.

If you don't like the plugin's default interface, just export the collected data as a CSV and print/use it somewhere else.

Everything is configurable, from the Web forms, to the format of the collected data.

Installation:

Unpack and upload it to the /wp-content/plugins/ directory.
Activate the plugin through the 'Plugins' menu in WordPress.
