The Popup contact form plugin comes with a WP backend settings page, a sidebar widget, a template tag and WordPress specific shortcode for embedding the form & popup in the site's frontend.
The form itself is AJAX driven, so whenever the user submits the form, the page won't get reloaded.
Installation:
Unpack and upload it to the /wp-content/plugins/ directory.
Activate the plugin through the 'Plugins' menu in WordPress.
