WP Contact Bank can be utilized for adding contact forms to a WordPress site.
To embed the form, just use the [contact_form] shortcode.
Additionally, a page in the WP backend allows the webmaster to select which users gets the emails sent through the contact form, or even configure a totally new address where these emails end up.
Installation:
Unpack and upload it to the /wp-content/plugins/ directory.
Activate the plugin through the 'Plugins' menu in WordPress.
