It extends the native Flag Module to allow users to enter descriptions when they flag a piece of content.
This can eventually be used to build a flag history.
It's possible to opt to keep old notes even when the content gets unflagged.
Installation:
Unpack in your modules folder (usually '/sites/all/modules/')
Enable under Administer > Site Building > Modules

- Flag nodes or comments for abuse providing a descriptive reason
- Flag users as friends leaving a hint
