TCPDB is a Web application to keep track of your IP addresses and hosts. TCPDB is suited for both small and large networks with many locations and subnets. Up to one A Class or 16777216 addresses is supported. TCPDB uses PHP and MySQL.
Version 3.8 adds a drop down box for choosing among many locations.

Windows 95/98/Me/NT/2000/XP/2003 Server, IIS/Apache with PHP and MySQL
