FreeMedForms is an open source Electronic Medical Record manager software created by a community of computer and medical workers.
Being written in Qt, FreeMedForms is a cross-platform software that runs on Linux, Mac OS X and Windows operating systems. It is also a Qt Ambassador project and compatible with both Qt4 and Qt5 toolkits.
